I hereby acknowledge and agree to the terms and conditions set forth in this Liability Waiver before receiving holistic body and movement services from [Essence of Motion].

Assumption of Risk: I understand and acknowledge that the holistic bodywork services provided by [Essence of Motion] involve physical touch and manipulation of soft tissues, joints, and muscles. I am aware that there are inherent risks and potential side effects associated with bodywork, including but not limited to soreness, bruising, and temporary discomfort.

Medical Clearance: I certify that I am in good physical health and have no medical conditions that would prevent me from receiving holistic bodywork services. I have disclosed any relevant medical history, including but not limited to allergies, injuries, or chronic conditions, to [Essence of Motion].

Informed Consent: I understand that the holistic bodywork services provided are not a substitute for medical treatment or diagnosis and are intended for relaxation and well-being

Communication: I agree to communicate openly about any discomfort or concerns I may experience during or after the session. I understand that [Essence of Motion] is not a licensed medical professional service, and any information provided is not a substitute for professional medical advice.

Release of Liability: In consideration for receiving holistic bodywork services, I hereby release, discharge, and hold harmless [Essence of Motion], their employees, and any affiliated entities from any and all liability, claims, demands, actions, or causes of action arising out of or in connection with the holistic bodywork services provided.

Photography and Recording: I grant permission for [Essence of Motion] to take photographs or record video/audio during the session for professional or educational purposes, ensuring my anonymity.

I have read this Liability Waiver and fully understand its terms. I voluntarily agree to its content and acknowledge that I am giving up substantial legal rights by signing it.
